Basic Contractor Profile Requirements

Provide 24/7 hour emergency service within 4 hours
Billing submitted within 15 days
All necessary licenses and insurance coverage

The contractor application is intended for the sole use of new contractors to USM. If you are currently providing services for USM please DO NOT submit an application. Only submit one Application. If you provide services that cross service offerings please fill out the application that reflects your primary business line and fill in other service offerings in the “additional services” section of the application.
